Book an appointment to register a birth

Before you start

You can use this form to book an appointment for yourself, or on behalf of somebody else, to register a birth.

This service is only available to:

• customers whose baby was born in North Yorkshire; or
• North Yorkshire residents.

A baby can only be registered at the appointment by:

• the natural birth mother; or
• the natural father if married to, or in a civil partnership with, the mother at the time of the baby's birth; or
• the second female parent if married to, or in a civil partnership with, the mother at the time of the baby's birth.

Only one person needs to attend the office to register the birth unless you were not married, or in a civil partnership, at the time of the birth in which case both parents will need to attend.

We accept credit or debit card payment for a birth certificate.

Before you complete this form, please make sure you have to hand:

• all relevant paperwork for when you visit; and
• details and availability of the person attending the appointment.
