You can use this form to book an appointment to register a death.
The death must have happened in North Yorkshire if you require a death certificate on the day of your appointment. The death must also have occurred no more than ten days ago to book online.
Before you complete this form, please make sure you have to hand:
Medical certificates are now forwarded to the registrars by email. Please ensure that you have received confirmation from your doctor or medical examiner that the medical certificate has been sent to the registrars before making your appointment. We will not be able to go ahead with the appointment without the medical certificate.
